Conditions of registration
1. To provide a service to adults with physical disabilities, learning disabilities, mental health issues and older people living in their homes.
Any other conditions unique to the service:
2. The support will be provided by 13 staff teams located at the addresses agreed during the variation dated 30 April 2019. The Care Inspectorate should be informed of any subsequent changes. The service will be provided by 2 staff teams based in Rothesay, Isle of Bute and 11 staff teams based in Oban.
3. The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details as it appears on this certificate.
4. The Registered Manager is also the Registered Manager for Affinity Trust - Argyll & Bute (CS2011290083)
